The method I use, which was learned and adapted whilst working with some pretty knowledgable people is as follows,

Use visual studio to generate simple web page content only - no styles or layout other than what you require in its natural order is included.  You can set style class names / skin names, and change a style class in code (e.g. because something should become emphasied).

You can then test functionailty.

Pass the web site over to the stylists.  They can edit the ASPX pages to get the correct look and feel.  If necessary, they can add in server side script (the same as, but rather than code behind) to add more fuctionality (e.g. page values come from a "dialog" web page).

If there is something that cannot be accomplished by the stylists, perform a change control and start back at the visual studio process.
